Navajo Public Health Nursing Program

Employment Type: Regular Full-Time
Work Schedule: Monday-Friday, 8:00 AM - 5:00 PM
Sensitive Position: Yes

DUTIES AND RESPONSIBILITIES

The Public Health Nurse provides comprehensive nursing services to individuals, families, and communities, with an emphasis on disease prevention, health promotion, and community-based care. Responsibilities include, but are not limited to:
  • Educate community members on healthy lifestyle choices, disease prevention, and medication management.
  • Develop and implement care plans for patients with chronic conditions such as asthma, diabetes, and hypertension.
  • Provide skilled nursing care and coordinate services with medical providers, including referrals for medical equipment and local support services.
  • Deliver prenatal and postnatal care, administer medications, and provide treatment for complex wounds in home and community settings.
  • Conduct follow-up for communicable diseases and consult with physicians as needed.
  • Review and assign referrals and submit quarterly reports related to disease surveillance and public health issues.
  • Collaborate with Community Health Workers to exchange patient information and coordinate care.
  • Conduct home health visits, including health assessments, medication monitoring, and arranging transportation for patients.
  • Provide instruction on the proper use of medical equipment and track referrals to ensure follow-up care.
  • Administer hearing and vision screenings, influenza vaccinations, and monitor patient immunization status.
  • Maintain and ensure proper use of assigned vehicles for community health visits.
  • Perform other related duties as assigned.
MINIMUM QUALIFICATIONS
  • Bachelor's degree in Nursing; and Two (2) years of experience in public health nursing; or An equivalent combination of education, training, and experience that provides the ability to perform the described duties.
SPECIAL REQUIREMENTS
  • Current licensure as a Registered Professional Nurse (R.N.) in the assigned state (Arizona, Utah, or New Mexico), or current multi-state licensure in a Nurse Licensure Compact state.
  • Within ninety (90) days of employment, incumbents holding multi-state licensure must obtain licensure in the assigned state (Arizona, Utah, or New Mexico).
  • Must possess a valid state driver's license.
  • Must obtain a Navajo Nation Vehicle Operator's Permit.
  • Must obtain Cardiopulmonary Resuscitation (CPR) certification within the required timeframe.
KNOWLEDGE, SKILLS, AND OTHER CHARACTERISTICS
  • Knowledge of principles and practices of public health nursing and community-based healthcare delivery.
  • Understanding of chronic and communicable diseases and their prevention.
  • Skill in providing patient education, counseling, and health promotion services.
  • Ability to engage patients with poor health practices and facilitate positive behavior change.
  • Experience delivering health education materials and conducting group presentations.
  • Proficiency in the use of specialized medical equipment to ensure quality patient care.
  • Experience managing ambulatory, acute, home health, and chronic care services.
  • Ability to plan, direct, and coordinate emergency medical care programs.
  • Strong interpersonal skills and ability to establish trust with patients, coworkers, and community partners.
  • Excellent verbal and written communication skills.
  • Ability to assess, identify, and respond to a wide range of health conditions affecting diverse populations.

PHYSICAL REQUIREMENTS AND WORK ENVIRONMENT

This position requires a moderate level of physical activity and extensive travel across the Navajo Nation to provide healthcare services to underserved populations. Key physical requirements include:

  • Physical Strength: Ability to lift and transfer patients and medical equipment, particularly in home care environments.
  • Endurance: Ability to stand and walk for extended periods during home visits and outreach activities.

  • Mobility: Ability to walk long distances, climb stairs, and navigate varied terrain.
  • Stress Management: Ability to manage challenging situations and diverse patient needs effectively.
