Job Description

Community Health Registered Nurse

If you believe healthcare is a right, that everyone deserves high quality care so they can enjoy their highest level of health and wellbeing, and you value each person's individual story consider joining us at Neighborhood!

We are seeking a Community Health Registered Nurse (CHRN) who is passionate about providing high quality primary healthcare to high risk populations, including those in our Healthcare for the Homeless program and other special populations. Do you know case/care management principles, do you find joy in working with the community and have an understanding of social drivers of health? This may be the role for you.

As a CHRN, you'll use your critical thinking, good judgement, and interpersonal skills as you identify alternative solutions or approaches to patient problems. You'll collaborate with clinical staff to design and implement meaningful and individualized patient-centered care. Essential duties include:

  • Build trusting relationships with patients to facilitate access to health care services and meet health needs
  • Engage patients to participate in health promotion, prevention, maintenance and restoration activities
  • Assist patients and families with accessing, navigating, and connecting with internal providers and services, outside community resources and programs, or other services as needed
  • Coordinate and revise the plan of patient care, as needed; identify and address any barriers affecting a patient's ability to adhere to a healthcare plan
  • Conduct community outreach, home visits, shelter/street outreach to identify people who are experiencing homelessness and other social support needs, and educate about health needs

You will be primarily based at our Mattina location, 300 Niagara Street in Buffalo, and will also travel to other Neighborhood sites on a regular basis.

The top three words employees say describe the work environment are: teamwork, supportive, kind. These are from an anonymous survey of Neighborhood employees for the Buffalo Business First Best Places to Work competition. Neighborhood has earned "finalist" distinction in the competition the last four years. We are a group of flexible and kind individuals who are open to each other's ideas, and see opportunities to innovate and find solutions when challenges arise.

Education and experience to be a Community Health Registered Nurse:

  • Degree from accredited 2 yr nursing school
  • Current NYS Professional Registered Nurse license
  • 2+ years clinical nursing experience
  • CPR certification
  • Attention to detail, organizational skills, and follow through
  • Knowledge of case/care management principles
  • Flexible with shifting priorities and timelines
  • Strong project and time management skills
  • Knowledge of Microsoft Office Suite, Adobe, and database management

What We Offer:

Compensation starts at $36.50 per hour.

Benefits: You'll have options for medical, dental, life, and supplemental insurance. We also offer a 403b match, health savings accounts with employer contribution, wellbeing programs, continuing education opportunities, generous paid time off and holidays.

RN Tuition Reimbursement Potential: Our status as a Federally Qualified Health Center (FQHC) means registered nurses have an opportunity for tuition reimbursement up to 60% repayment of qualified student loans directly related to a nursing degree. RNs who are granted reimbursement are required to fulfill a two year, full time commitment to maintain eligibility. The reimbursement program requires an application and is not guaranteed.

About Neighborhood: Neighborhood Health Center is the largest and longest serving Federally Qualified Health Center in Western New York, and is the highest ranked health center for quality in the region. We provide primary and integrated healthcare services all under one roof, regardless of a person's ability to pay. Services include internal/family medicine, pediatrics, OB-GYN, dentistry, podiatry, psychiatry, vision care, nutrition and behavioral health counseling, and pharmacy services. We're working toward a Western New York where all enjoy their highest level of health and wellbeing.

Neighborhood Health Center is an Equal Opportunity Employer.
