Job Summary:

We are seeking an experienced and detail-oriented MDS Coordinator RN to manage and oversee the assessment and care planning process under the MDS 3.0 system. The MDS RN ensures accuracy in resident assessments, supports clinical documentation, and helps maximize reimbursement through proper RAI (Resident Assessment Instrument) completion and submission.

Key Responsibilities:

  • Coordinate and complete the MDS assessments as per state and federal regulations.
  • Ensure accuracy and timeliness of all OBRA and PPS assessments.
  • Collaborate with interdisciplinary teams (IDT) for care planning.
  • Monitor and audit documentation to support MDS coding and Medicare eligibility.
  • Submit MDS data to CMS through appropriate software (i.e., CASPER/QIES).
  • Participate in case-mix, utilization review, and quality assurance processes.
  • Train nursing staff on proper documentation practices.
  • Track and follow up on significant change assessments.
  • Maintain current knowledge of RAI guidelines, PDPM, and regulatory updates.

Requirements:

  • Current and valid Registered Nurse (RN) license
  • Minimum 1–2 years of experience as an MDS Coordinator or in long-term care nursing.
  • Strong understanding of MDS 3.0, OBRA & PPS assessments, PDPM .
  • Proficiency in PointClickCare , MatrixCare, or other MDS software systems.
  • Excellent attention to detail, analytical, and organizational skills.
  • Ability to work independently and within a team environment.

Preferred Qualifications:

  • RAC-CT certification (Resident Assessment Coordinator–Certified) is a strong plus.
  • Experience with Medicare/Medicaid reimbursement systems.
  • Strong communication and leadership skills.
