Reports To :  Director of Social Services 

Status : 80 hours EXEMPT per two week pay period (possible part-time if requested)

Shift : 8:00 am – 4:30 pm 

Job Schedule : Full Time

Weekly Hours : 40 hours 

Remote Position : No

Job Summary 

HRMC is seeking a self-motivated, resourceful and forward-thinking individual to join our Social Services team.   Works with the Social Services Director to provide exceptional social services consulting for HRMC.

Qualifications: (Minimum Education and Experience)

• MSW required.
• Licensed by the South Dakota Board of Social Work Examiners.
• Visual, auditory and speech acuity requisite to verbal and written communication with staff, patients, families, physicians and other persons with whom contact is part of the work activities.
• At least two years' experience in community social work, 1 of which was in a dialysis unit (preferred). 

Physical Aspects of the Job/Essential Physical Demands :

• Work is indoors in clean, well-lighted, climate controlled environment with minimal hazards under normal conditions.  Works in patients' homes, hazards subject to condition of patients' homes.
• Involves travel to patients homes.
